He was predeceased by: his parents, Andrew Lebak and Viola Lebak. He is survived by: his wife Nancy Lebak; his siblings, Jim Lebak and Joan Keck; his mother-in-law Helen Hinkle; his siblings-in-law, Susan Asay, John Asay and Valerie Hinkle; his children, Kimm Rapp (Wilhelm) and David Lebak (Kimberly); his grandchildren, Melanie Griggs (Austin), Adam Rapp (Morgan), Stephanie Burke (Bill), Julianna Lebak and Andrew Lebak; his great grandchild Bill Burke; and his brothers-in-law, Harry Hinkle and Bill Keck. He is also survived by many nieces, nephews and friends.
