He was predeceased by: his parents, Francis Gregory Forsyth, Sr. and Bernice V. Murphy Forsyth. He is survived by: his wife Alice Forsyth; his siblings, Kathryn Forsyth (Edward F. Liston, Jr.), Frank Forsyth (Carla) and John Forsyth (Julie); and his brothers-in-law, Michael Estler (Kate, Timothy Estler) and Joshua Stroh. He is also survived by many cousins and other relatives.

In lieu of flowers, donations to Habitat for Humanity would be appreciated by Patrick's family.
