He was predeceased by: his significant other Rita. He is survived by: his daughters, Rosanna Balsamo and Donna Kozak; his grandchildren, Jena Siegrist (Kevin), Domenica Oliveira (Marcelo) and Anthony Kozak (Kelly); and his great grandchildren, Colin, Dominic Siegrist, Marc and Antonia Oliviera.

In lieu of flowers a memorial donation in Dominick's name may be made to the Lupus Foundation.
