She was predeceased by: her husband Milan; and her sister Doris Ilovichny. She is survived by: her sons, Richard Slavik of California, MD and David Slavik (Margaret) of Dumont, NJ.; her grandsons, Michael (Janelle) and Christopher; and her niece Susan Farley.
