He was predeceased by: his father Sidney Forman. He is survived by: his mother Alice Perlmutter Forman; his wife Donna Fioravanti Forman; his daughters, Jaclyn and Jessica; and his siblings, Cathie Forman, Debbie Forman and Steven Forman. He is also survived by twelve nieces and nephews.
