She was predeceased by: her parents, Joseph Sweitzer and Ann Sweitzer. She is survived by: her husband John Hlavacek; her children, Michael Hlavacek, Brett (Kim Hlavacek) and Rhonda Hlavacek; and her sisters, JoAnne Amery, Kathy Roll and Barbara Sweitzer. She is also survived by nieces and nephews.
In lieu of flowers the family requests that donations be made to The American Diabetes Association.
