He was predeceased by: his son Michael Boutwell; and his parents, Howard Boutwell and Mary Boutwell (Parrot). He is survived by: his wife Shirley Boutwell (Murby); his children, Brenda Shuttle (Boutwell) and David Boutwell Jr.; his siblings, James Boutwell and Judi Esola (Boutwell); and his grandchildren, Cody Boutwell, Morgan Boutwell, Nicole Conceicao (Boutwell), Danielle Boutwell, Shannon Shuttle and Tyler Shuttle. He is also survived by his great grandchildren, cousins, nieces, and nephews.
