She is survived by: her husband Dave; her children, Dustin Berwick (Crystal), Katelynn Hazelton and Annie Gilbride (James); her father Gerald; and her sisters, Vicki Sweat (David) and Anna Perkins (Jacob). She is also survived by her grandchildren, aunts, uncles, nieces & nephews.
