He was predeceased by: his father Arthur Cook. He is survived by: his wife Kelly Cook of Keene, NH; his sons, Ben Cook (Shelby Redfield) of Boston, MA and Walter Cook of Keene, NH; his brother Kevin Cook (Pamela Marin) of Northampton, MA; and his sister-in-law Leslee Newport (Howard) of Noblesville, IN. He is also survived by several nieces, nephews, cousins, and may close friends.
