Cook and Mabel Casey Cook; his wife Gayle Neily Cook; his children, Donald Jr. (Sara), Kevin (Terri), Christine and Kerry; his grandchildren, Casey, Nathaniel (Erica), Courtney (Ed), Donald III, Rachel (Dominic) and Joseph; his great grandchildren, Edward, Eva and Amelia; and his siblings, Richard (Kathy) and Marjorie. He was also cherished by many nieces and nephews.
