He is survived by: his children, Michael L. Ellsworth (Mary-Jane) and Diane V. Holley (James H. Holley); his grandchildren, Jamie L. Walker (Holley), Lisa M. Lancaster (Fales), Charles Carlyle, Freddie M. Parrott (Trisha A.), Christian C. McCullough, Jessica J. T. Ellsworth and Norman Faucher; and his great grandchildren, Kai, Cullen Walker, Payton Soltys, Isabella, Madilyn Travis, Jace Parrott, Jaydon, August and Delilah Rose Anne Faucher.
In lieu of flowers memorial donations may be made to St. Jude Children's Research Hospital.
