He was predeceased by: his parents, John Machacek and Mayme Machacek (Ohnoutka); his siblings, John and Adelia Dolezal; and his brothers-in-law, Henry Dolezal, George Pabian and Tom Roberts.
He is survived by: his wife Marilyn; his sister-in-law Bonnie Roberts; and his nieces and nephews, Debbie Crosby, Donna Stara, Denise Dolezal, Robin Seymour, Carol Pelan, Penny Brian, Jeff Pabian and Duane Dolezal.
