He was predeceased by: his parents, Dareld Blake and Joyce Blake; his niece Missy Blake; and his nephew Jon Lemmerman. He is survived by: his wife Shari; his children, Ashley Johnson (Joe) of Sutherland, Justina Blake (Nick Eyten) of North Platte, Zachary Blake (Ali) of Sutherland and Preston Blake of Cozad; his grandchildren, Dean, Jordan, Josephine and Nash; and his brothers, Randy Blake (Linda) of North Platte and Brad Blake (Angie) of Marquette.
In lieu of flowers, memorials are to St. Jude Children's Research Hospital or KJLT Radio.
