He was predeceased by: his parents, Fred Polacek and Sophie Polacek (Dobesh); his son Michael Polacek; and his sister Selma Schmit (Dwaine). He is survived by: his wife Evelyn Critel of Surprise; his son Brian Polacek (Traci) of Sac City; his daughter-in-law Judy Formanek (Rob) of Bennington; and his grandchildren, Casey Polacek (Beka), Jenna Colletti (Vince) and Will Polacek (Annie Henry). He is also survived by two great-grandchildren.
