He was predeceased by: his parents, Ludvik Anton and Mary Ann Policky (Coufal); his sons, Ron and Charles; and his siblings, Ludvik (Georgia Policky), Emil (Rose Policky) and Lillian Tlamka.
He is survived by: his close friend Louie; and his children, Alan Policky, Jim Policky, Diane Palmer, Judy Policky and Joe Policky. He is also survived by several grandchildren, numerous great-grandchildren; numerous other nieces and nephews.
