Kozak of Omaha, NE, Timothy P. Kozak (Nicole) of Plattsmouth, NE and Tracey Kozak Sr. (Natalie) of Plattsmouth, NE; his grandchildren, Ashley Renshaw, Elizabeth Hadraba (Jordan), Timothy Kozak, Logan Kozak, Frederick Karnes Jr. (Alix), Drew Karnes (Brianna), Tracey Kozak Jr. and Jozlynne Kozak; his great grandchildren, Landyn, Savannah, Luna, Aria and Everleigh; and his siblings, John Kozak Jr. of Omaha, NE, Wesley "Wes" Kozak of Omaha, NE, Michael Kozak of Omaha, NE, Maria Kozak of Omaha, NE and Stephanie Massie (Michael) of Council Bluffs, IA. He is also survived by numerous nieces, nephews, and cousins; and many friends.
