He was predeceased by: his parents, Frank Forman and Mary Jane Forman (Welna); his grandson Finn Moeller; his nephew Scott Forman; and his sister-in-law Debbie Forman.
He is survived by: his wife Marlyss; his daughters, Tracy Forman and Elizabeth Erven (Tom); his stepson Kyle Moeller (Kim); his grandchildren, Thomas J. Erven, Gannon, Hudson, Brody and Wren Moeller; and his siblings, Ron Forman (Margaret) of Byron, MN, Tom Forman (Lenanne) of Omaha, Janet Lee (Dan) of Bellevue, Cindy Forman of Omaha, Kevin Forman (Sally) of Mesa, AZ and Jeanne Rich of Omaha. He is also survived by many nieces and nephews, other relatives and friends.
Celebration of Gary's Life: Saturday, Jan. 5, 2019 at 10:30am at Citylight Church (West Location, 3401 Oak View Dr.) In lieu of flowers, memorials given to the family will be used in Gary's memory at a later date.
