He is survived by: his father Rich Cook; his children, Shanandoah Scott (Tim), Breanna Cook (Tyson Courter), Trevor Cook (Naomi), Damon and Brendin; and his sister Suzette Casillas. He is also survived by several grandchildren and great-grandchildren.
