He was predeceased by: his parents, Alfred Czuba and Donna Czuba (Olcott); and his brother Jon. He is survived by: his siblings, Jay Czuba of Columbus and Janelle Ryan (John) of Columbus; his niece Jessica Hasanbegovic (Mirza) of Omaha; his great-nieces and great-nephews, Addison, Maxon and Oliver; his niece Jennifer Ryan of Omaha; and his nephew Josh Ryan.
