She was predeceased by: her parents, Harvey Hofman and Ramona Hofman; and her grandparents, William, Henrietta Hofman, Reinhold and Margaret Hinz.
She is survived by: her children, Tessa Hlavaty Thomsen (Jared) of Portland, Oregon and Troy Hlavaty of North Platte; her boyfriend Michael Strombeck of Simi Valley, California; her former husband Todd Hlavaty of North Platte; and her siblings, Brent Hofman (Debra) of Indianola and Peggy Stoltenberg (Cal) of Norman, Oklahoma. She is also survived by many nieces and nephews.
Following Tammy's wishes after her death, she continued to save lives through the gift of organ donation.
