He was predeceased by: his parents, Alex Kuszak and Rose Kuszak (Woitalewicz); his wife Norma Michalski; and his siblings, Edwin, Ernie, Clemence, Louie, Dominic, Ben Kuszak, Lordine Budzinski and Rita.
He is survived by: his children, Ken Kuszak (Bernadette) of Geneva, NE, Alan Kuszak (Sheryl) of St. Paul, NE, Randy Kuszak (Teresa) of Ashton, NE and Debbie Mason (Randy) of Grand Island, NE. He is also survived by nine grandchildren; twenty-one great grandchildren and seven great-great grandchildren.
