He was predeceased by: his wife Helen; his parents, Charles and Emma; and his brother Kenneth. He is survived by: his sister Audre Ferrier (Dvorak); his children, Carla Lash (Ed) of Gallup, N. M., Dennis Dvorak (Leah) and Bruce Dvorak (Lincoln); his grandchildren, Eddie, Makdes Lash, Adam Dvorak (Brittney), Carrie Dvorak, Kate, Anna and Lina Dvorak; and his great grandchild Emersyn Dvorak.
