He was predeceased by: his parents, Frank Kuzelka and Marie Kuzelka; and his brothers, Ronald and Allen Kuzelka. He is survived by: his wife Eileen; his children, Jeffrey (Amity) and Amy; and his granddaughters, Lacy Helms (Mike) and Jordyn Corn.
