He was predeceased by: his parents, Frank Sr. Kosmacek and Mary Kosmacek (Lukas); his wife Marie Helen Myers Day; and his siblings, Joe, Rudy, Mary Alice Witte and Agnes Haba. He is also survived by Many Nieces & Nephews.
In lieu of flowers, memorials may be given to American Cancer Society or to the family for a memorial to be established at a later date.
