He is survived by: his former wife Darlene Ostendorf; his sons, Bradley and Jeffrey; his former wife Cathy Cook-Harris of Elwood; his sons, Mike Underwood of Cozad, Brad Underwood (Amber) of Kearney and Jeff Underwood of Cozad; his stepchildren, Sebrina Berumen (Michael) of Aurora and Shon Harris (Jan) of Lula, Georgia; his brothers, Kenny Underwood (Barb) of Holdrege, James Underwood (Lois) of Lexington, Rod Underwood (Lois) of Lexington and Jerry Underwood of Wilbur; and his siblings-in-law, Christy Underwood of Azle, Elsie Underwood of Lexington, Virginia Wind (Charles) of Mesa, Arizona, Lynn Cook (Joyce) of Elsie and Tom Cook of Mullen.
