He was predeceased by: his parents, Anton Chvatal and Irma Chvatal (Ondracek); his wife Gladys R. Prochaska; his sister Maxine Bolte; and his great grandchild Grant Slattery.
He is survived by: his children, Karen Michaelson (Mark NewVille), Linda Brower (Mark), Donna Schuetz (Jon), Rose Suddarth (J. B.) and Terry Chvatal (Vicki); his grandchildren, Annie Slattery (David), Ryan Schuetz, Matt Schuetz (Alyssa), Jamie Wagner (Reid), Josie, Erin, Julia Lee, Drake, Blair Suddarth, Jacob and Logan Chvatal; and his great grandchildren, Parker, Cally, Grayson Slattery, Isaac and Luca Schuetz. He is also survived by many nieces and nephews.
