We are sad to announce that on July 8, 2022, at the age of 89, Rita M Polak (David City, Nebraska), born in Prague, Nebraska passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

She was predeceased by: her parents, Emil Cernohlavek and Kristina Cernohlavek (Osmera); and her husband Dick. She is survived by: her children, Diana Kucera (Bill) of Windsor, CO, Dr. Mark Polak (Deb) of Virginia Beach, VA, Joan Stewart (Dale) of McCook, NE, Karen Johnson (Randy) of Windsor, CO and Ann Reisinger (Nick) of Winfield, MO.. She is also survived by 14 grandchildren, and 13 great-grandchildren.
