He was predeceased by: his parents, Raymond Kozisek and Albina Kozisek (Rerucha); his wife Joan Leitgeb; and his brothers, Edwin and Raymond.
He is survived by: his children, Lynn Barnes of Aurora, Mark Kozisek (Stacey) of Gretna and Michael "Mick" Kozisek (Shawna) of Papillion; his sisters, Gladys Kozisek of Lincoln, Georgianne Mastera (Jim) of Lincoln and Pat Foral of Brainard; and his grandchildren, J. C. Barnes, Brenna Barnes, Nathan, Noah Kozisek, Jenna and Leah Swaney.
The family suggests memorials in lieu of flowers to david place in David City. They request no flowers, plants or statues at this time.
