He was predeceased by: his parents, Frank E. Slezak and Ruth M. Slezak (Domling); his wife Irma Jean Loukota; his brother Roger; his brothers-in-law, Emil Steinacher and Gary Frankforter; his sister Darlene Vodicka; his nephew Rick Krupicka; and his sister-in-law Sylvia Slezak.
He is survived by: his son Mark; his daughter-in-law Twila Slezak of Bartlett, TN; his daughter Barbara Slezak of Crete; his siblings, Norman (Harriett Slezak), Virginia, Shirley Steinacher of Geneva, Evelyn (Charles Krupicka), Linda (Henry Lopez), Judy (Douglas Turek), Franklin "Butch" (Karla Slezak) and Beverly (Loren Filipi); and his brother-in-law Darrell Vodicka of Geneva. He is also survived by 6 grandchildren, 5 great grandchildren and many nieces and nephews.
In lieu of flowers, memorials may be given to the Milligan American Legion.
