He is survived by: his wife Deb Eppenbach; his stepsons, Troy Eppenbach (Leah), Justin Eppenbach (Kelli) and Matt Eppenbach (Janet); his grandchildren, Larissa, Tessa, McKenna, McKenzi, Maddox, Easton, Graham and Collins; and his siblings, Ken Polacek (Judy), Dave Polacek (Connie), Marvin Polacek (Deb), Mary Ann Briza (Reggie), Richard Polacek (Jane), John Polacek (Holly), Dennis Polacek (Darla), Tom Polacek (Jana), Barb Byers (Steve), Bob Polacek (Francine) and Jim Polacek (Elly).
