He was predeceased by: his father Emil W. Kouba; his sister Teresa Rountree; his grandparents, Emil, Rose Kouba, Douglas and Emily Cavett; and his brother-in-law Stephen Rountree.
He is survived by: his wife Rita Booze of Bruno; his mother Patricia Kouba (Cavett) of David City; his brothers, Emil D. "Doug" and Dan Kouba of David City; and his children, James Kouba (Emma) of Vermillion, Rebecca Zulkoski (Ross) of Columbus, Amanda Kouba (Tyler) of Boise and Sarah Naber (Chris) of Weston. He is also survived by 11 grandchildren, and another grandchild expected in March, several nieces and nephews.
