He was predeceased by: his parents, Raymond Dvorak and Helen Dvorak (Kucera); his son Curtis Dvorak; his daughter-in-law Dawn Dvorak; and his sister Bernice Eckhardt.
He is survived by: his wife Leona Prochaska of Brainard; his sons, Jeff of Brainard and Dallan Dvorak (Janine) of David City; his grandchildren, Devon Tonniges (Eric), Jared, Julia, Justin Dvorak and Jaelyn Dvorak; and his great grandchildren, Kollyn and Graham Tonniges. He is also survived by several nieces and nephews.
