He was predeceased by: his grandparents, John and Dorothy Slowik; and his uncle Mike Slowik. He is survived by: his wife Susan F. Kemmish; his children, Patricia and Johnathan; his siblings, Jeremy and Sarah Slowik; his parents, John Slowik and Denise Slowik; his grandmother Shirley Tingley; his uncles, Donald Slowik (Cindy), David Slowik, Gary Slowik and Kavin Tingley; and his aunt Susan Slowik. He is also survived by nieces, nephews, family and friends.
