He was predeceased by: his parents, Daniel C. Lee Sr. and Mary Jane Stekar Lee; his sister Ellen Gingras; his brother-in-law Vincent Saverino; and his nephews, Antonio Saverino and Scott Forman.
He is survived by: his wife Janet Forman; his sons, Aaron (Gina), Adam (Kelly), Jacob (Andrea) and Joshua; and his sisters, Dorothy Saverino, Julie Fellows (Larry) and Cecilia Brown (Ralph). He is also survived by 18 grandchildren, many nieces and nephews and life-long friends.
