She was predeceased by: her parents, Joe Forman and Evelyn Forman; and her sister Marge Vanek. She is survived by: her husband Wally; her son Joe Kozicki (Tara); her grandson Vincent Kozicki; her sister Bev Riha (Vic); and her brother-in-law Dennis Vanek. She is also survived by five nephews.
