He was predeceased by: his parents, Robert Cook and Irene Cook; his nephew Troy Cook; his sister Janith Ness (Jim); and his brothers-in-law, Jim Miller and Ken Hoffman.
He is survived by: his wife Shirley; his child Bobbie Slaughter (Terrill); his grandchildren, Baillie, Jordan Lyste (Mike), Julia and Sydney; his daughter Becky Gildea (Jeremy); his grandchildren, Madison and MacKenzie; his son Matthew Cook (Mandy); his grandchildren, Kylie and Carter Cook; and his siblings, Jim Cook (Bev), Cherryll Hoffman, Jerry Cook (Ione), Kathryn Miller, Lowell Cook (Joanne) and Doris Mancell (Mark). He is also survived by many nieces and nephews.
