He was predeceased by: his parents, Thad and Frances; and his brother Norman. He is survived by: his wife Juneve Givers; his children, Amity Cook (Brad) and Jeremiah Givers (Jenna); his grandchildren, Andy Cook, Logan Cook and Elliot Givers; his brother Ted Givers (Joyce); and his nephew John Givers. He is also survived by relatives and friends.
