He was predeceased by: his grandparents, Richard, Dorrene Konzak and Gordon Berg; his aunt Cindy Berg; and his cousin Amie Konzak.
He is survived by: his parents, Dale Berg and Stacey Konzak (Berg); his sister Erin Berg; his brother-in-law James Gibbens; his nieces, Eden and Nora; his grandmother Beth Berg; his aunts and uncles, Doug (Joan Konzak), Diane (John Kuchera), Karen (Michael Tiradani), Sidney (Sam Whitehead), Kim (Lee Jones), Arne (Kris Berg), Dean (Carol Berg) and Karn (Les Windjue); and his pet Paxton. He is also survived by many other cousins and their children.
Memorials may be directed to the family for their use or to the following organizations in memory of Andrew. American Suicide Foundation, TEARS, Alzheimers Foundation or the American Heart Association.
