He was predeceased by: his parents, James Dvorak and Marie Dvorak (Maresh); and his brother Gordon. He is survived by: his former wife Rita Mattson; his wife Mary Phipps (Flaaskog) of Grafton, ND; his children, Renae McMartin (Randy), Lorie Burns (Anthony) of Waunakee, WI and Jeremy Dvorak (Danielle) of Grafton, ND; his stepdaughter Codi Johnson (Dan) of Vero Beach, FL; his grandchildren, Alyson, Trevor, Courtney, Cody, Kimberly, Aiden, Paetyn, Abby, Taylor, Jayla, Luca and Rowan; his great grandchildren, Ellis, Ila Belle, Keira, Brayden and Inesza; and his siblings, Anna Mae Hankey (John) of Park River, ND, Eileen Bennett of King City, CA, Beverly Hanson (Russell) of Watford City, ND and Rita Atkinson (Mike) of Paynesville, MN. He is also survived by several nieces and nephews.
