He was predeceased by: his parents, Arvel Dean Cook and Lola Edith McGuire Cook; his wife Rose Marie Cook; his sister Elaine Greene; and his nephew Randy Greene.
He is survived by: his children, Lisa Cook of Butler, Tennessee, Bill Eldreth (Rita) of Taylorsville, Jeff Cook (Lisa) of Zionville and Ricky Cook; and his sister Diane Terry (Steve) of Zionville. He is also survived by eight grandchildren and six great-grandchildren.
