He was predeceased by: his parents, Curtis Lee and Hazel Cheek Smith. He is survived by: his siblings, Sandy Toney (CW), Ricky Smith, Michael Smith (Cindy) and Dennis Smith (Tracy); his wife Tina Willard; and his close friend Abby. He is also survived by several nieces and nephews.

In lieu of flowers, the family requests that donations be made to the funeral home to assist with funeral expenses.
