He is survived by: his wife Shirley Lee Prather Lewis; his daughters, Roxanne Hawks (Jack), Crystal Salmon (Craig) and Teresa Lewis; his grandchildren, Daryl Hawks (Julia), Matthew Hawks (Jennifer) and Amber Lambert (Matthew); his great grandchildren, Gabriel Lambert, Levi Lambert, Isaiah Lambert, Judah Lambert, Rachel, Liam Hawks and Dahlia Hawks; his brother James "Jim" Lewis (Jonell); and his sister-in-law Marie Prather.
