He was predeceased by: his parents, Clay Hayworth and Vada Hayworth. He is survived by: his wife Eldene Hayworth; his children, Larry Dale Hayworth and Susan Renee Tollison; and his sister Jane Brendle. He is also survived by four grandchildren; and three great grandchildren.
