He is survived by: his wife Donnie Hooks Formy-Duval; his sons, Coval Formy-Duval of Whiteville, NC and Pierre Formy-Duval (Julia) of Tabor City, NC; his granddaughter Isabella Formy-Duval; his stepsons, John Hooks (Karen) of Wilmington, NC and Phillip Hooks (Kristen); and his brothers, Jack Formy-Duval (Faye) of Chadbourn, NC and Gene Formy-Duval (Joyce).
