He was predeceased by: his parents, Bobby Junior Faulk and Lina Dale White; and his sister Melanie Dale Faulk. He is survived by: his wife Crystal Simmons Caswell of Whiteville, NC; his son Zachary Scott Faulk of Whiteville, NC; his sister Jo Dean Spivey of Ash, NC; and his nephew Derrick Faulk of Whiteville, NC. He is also survived by several cousins, Aunts and Uncles.
