He was predeceased by: his parents, Percy Slade and Ann Slade. He is survived by: his children, Jamie Lee Slade Bailey (Brian), Michael Slade, Christopher Slade and Jonathan Slade (Miranda); his grandchildren, Tripp Bailey, Wyatt Bailey and River Slade; and his siblings, Gala Slade LeGrand (Keith), Eric Slade (Kellie) and Amy Slade Lanier. He is also survived by nieces, nephews, aunts, uncles and cousins.

In lieu of flowers, memorial gifts and contributions in memory of Perry may be made to Beulah Hill Baptist Church - PO Box 492 Pinehurst, NC 28374 and Sandhills Children's Center - 1280 Central Drive, Southern Pines, NC 28387.
