He is survived by: his mother Claudia Lynn Smith; his wife Jennifer Elliott Miller; his children, Dannion Hollar and Jewel Castonguay; his parents-in-law, Jean Elliott and Gerald Elliott "Elyse"; his siblings, Joseph Miller, Jonathan "Megan" Smith, Ashley Robinson and Angela "Matthew" Taylor; his sisters-in-law, Rebecca Elliott "Thom" and Melissa Elliott; his stepsister Samantha Miller; and his nieces, Abbie Taylor, Emily Robinson, Alexia and Hannah Jones.

In lieu of flowers, memorial contributions in Jeremy's name may be made to the family to assist with funeral cost.
