He was predeceased by: his father Frank Sulkowski, Jr.. He is survived by: his wife Heather Sulkowski; his daughter Doxie Brynn Sulkowski; his mother Gloria Sulkowski; his brothers, Joseph Sulkowski and Frank Sulkowski III (Lori); and his nieces, Abigail, Aubrey, Charlotte and Emma.
