He is survived by: his wife Kathy Hawks; his sisters, JoAnn Boger and Linda Potts; his son Larry McIntosh (Sherry); his grandchildren, Eric McIntosh (Faith) and Sarah Burchfield (Randy); his great grandchildren, Ethan, Samuel and Julia Burchfield; and his stepmother Gladys S. Hawks.
