He is survived by: his wife Gloria Polk; his children, Barbara Edgeworth (Aaron), Evelyn McLendon, Sheila Polk (James), Nelson McLendon and Jerry Polk II (Tonya); his grandchildren, Desiree Colston, Dominique Hammonds, Aaron Edgeworth, Malachi Knotts, Emmanuel Knotts, Chloe Colston and Faith Polk; his siblings, Lillie Watkins (Sherman), Minnie Sturdivant, Velvet Sturdivant of Polkton, NC, Catha Coleman (Freddie), Jimmy Polk (Rosalind) and Ricky Polk (Karen); his sisters-in-law, Marie Polk of Morven, NC, Sara Butler and Mable McLendon, MD; and his close friend Sherman McLendon. He is also survived by nieces and nephews, cousins and friends.
